A.「スマート伝送」のご利用を終了するには、「スマート伝送解約届」および委任期間の終了日を記載した委任状のご提出が必要です。~

下記よりダウンロードし、必要事項を正しくご記入の上国保連お届印を押印して弊社までごご提出ください。~
